How to fill the form

  1. Click the green "Submit as Organization" button or the blue "Submit as Individual" button below, whichever applies to you. Gmail opens with a ready email. The To address, the Subject and all the answers are already filled in. Sign in to Gmail if it asks. The email is sent from your own Gmail account.
  2. Fill in the mandatory details marked with * at the top of the email, next to each field:

  3. Please read all the guideline answers carefully before submitting.
  4. After filling in these details, click Send (the blue button at the bottom left of the email). Blue Send button in Gmail
